The Etiwanda School District will begin pre-registering kindergarten students
for the 2012/2013 school year at each of our elementary school sites on:
Wednesday, March 28, 2012 beginning at 9:00.a.m.
The following items are required to complete your
child's registration (this information applies to kindergarten as well as new students in
all other grade levels):

A certified birth certificate

Immunization record
-
Proof of residency - Present two items from the following list
below:

• Gas, electric, water, trash or telephone bill (land line, no cell
bills) ,
cable bundle package (complete bill w/digital phone listed on bill)
• Escrow papers (Need certified copy upon closing)
• Homeowners/renters insurance policy,
• W-2 form (from most current tax year)
Your child must be five years of age on or before November 1,
2012 to begin school in August 2012.

Children entering kindergarten are required to have

2 doses of Measles and 1 dose of Mumps, 1
dose of Rubella ALL must be given on or after the 1st

Students will not be placed in
classes unless their immunization requirements are up-to-date. |

proof - Information on this required immunization for
students entering 7th and 8th grade (and available clinics) can be found
here.
California law, Education Code Section 49452.8, now requires that your
child have an oral health assessment (dental check-up) by May 31 in
either kindergarten or first grade, whichever is his or her first year
in public school.

If you are in escrow on a home within the Etiwanda School District and
would like your child to attend the school of residency for your new
address, you must submit an inter or intra district transfer and provide
escrow papers, with the estimated closing date, to the CWA Department at
Etiwanda School District office – 803-3138. |
